Figures obtained by the Lib Dems show over 700 incidents of data loss at health boards between 2009 and 2013, and 360 incidents reported by councils in the same period.
Incidents at the Scottish Government included confidential papers being lost in transit, sent to wrong addresses and personal information being wrongfully disclosed.
In one case an application was submitted to Mental Health Tribunal Scotland by a medical officer with an incorrect address for the named person and this led to confidential papers being sent to the wrong address.
Scottish Liberal Democrat leader Willie Rennie warned the losses prove the dangers of plans to create a new Scottish Government database of citizen information which 120 Scottish agencies and organisations could have access to.
Mr Rennie said: “These figures should make SNP ministers pause for thought. The SNP’s plans to extend access to the database to 120 Scottish organisations only extends the risk of data loss taking place on a seismic scale. It is intrusive, expensive and we have no guarantees that our personal information would be kept safe.”
